Community Concepts is seeking a Home Visitor to work with families with infants/toddlers and expectant parents. This position is 40 hours and year-round.

Starting rate: $19.30/hr

Essential Duties:

Partner with families by supporting parenting skills within the home environment
Monitor children’s developmental progress
Collaborate with families to establish a source of continuous, accessible, coordinated developmental, health, and dental care
Ongoing documentation and maintaining records, as well as data collection
