Join the Catholic Charities Maine Behavioral Health Network in Portland, Maine, and help create positive change! We currently have one full-time opening for a Transitional Care Coordinator.
The Behavioral Health Network is dedicated to helping adults and families struggling with a mental health diagnosis and/or substance use disorders. We are an interdisciplinary team of psychiatrists, nurse care managers, clinicians, care coordinators, and peer support specialists who provide an integrated and mission-guided approach to client care for adults and families.
The BHN Transitional Care Coordinator shall be a trained, supervised professional who will provide Care Coordination Services within the Client Access Response and Engagement (C.A.R.E.S.) Team program. Other Key Responsibilities include:
- Act as a collaborative bridge for individuals enrolled in all BHN programs.
- Support a caseload of individuals needing assessment, stabilization, or help navigating life and service transitions.
- Deliver direct support for:
- Service requests
- Pre-intake and orientation
- Access to services
- Outreach activities
- Ongoing case support
- Ensure compliance with state, agency, and program guidelines.
- Provide support to individuals and families as part of the BHN and C.A.R.E.S. team.
- Maintain professional training and supervision as required for the role.
